I was able to tour the independent living and assisted living. I would give the staff a 4 because I had to do a lot of follow-up with her. I did not try the food, but I did see the different restaurants and the dining halls. I would feel comfortable putting my mother there if they had the services that she needed.
RBC transfusion should only be considered when the hemoglobin concentration is less than 7 g/dL. Platelets should be transfused when the count is 50×10⁹/L during ongoing PPH (Muñoz et al., 2019). Initial fluid replacement with a balanced crystalloid solution is recommended . The client will have a lochia flow of less than one saturated perineal pad per hour.
Complications of Postpartum Hemorrhage
This enhances the effect of uterotonic agents on myometrial tissue by facilitating the emptying of the uterine cavity (Koh et al., 2020). The degree of uterine contractility will measure the status of the blood loss. Uterine atony allows the blood vessels at the placenta site to bleed freely and usually massively because the muscle fibers that compress the bleeding vessels are flaccid. Large venous areas are exposed after the placenta separates from the uterine wall and bleeding is controlled by the contraction of smooth muscles in the uterus. The best safeguard against uterine atony is to palpate the client’s fundus at frequent intervals to ensure her uterus remains contracted.
My mother was put in a wheelchair that had dried vomit caked on the side. When I asked another wheelchair they seemed bothered by my request. People were sitting in wheelchairs in hallways asking me to push them to different rooms. Others called out for help but nobody was listening to them. I understand it is not the job of the nursing staff to provide entertainment to the home's lonley residents, it disturbed me to experience the apathy the staff seemed to have.
How PHH will change the healthcare industry
Nevertheless, a blood loss greater than 500 mL in a vaginal delivery should be considered abnormal (American College of Obstetricians and Gynecologists , 2017). The most common cause of PPH is uterine atrophy, which is when the uterus does not contract following delivery of the placenta, leading to abnormal blood loss. If not recognized and corrected promptly, the mother may experience shock and death.
Most healthcare providers include an order for catheterization to prevent delaying this corrective measure. After the uterus is firm from massage, the bladder should be emptied to keep the uterus firm. Short-term rehabilitation helps residents recover from a hospital stay. It provides individualized rehabilitation and nursing therapy for accidents, strokes, hip fractures, joint replacement and other skilled nursing needs.

My mother has her own meal plan where she could eat three meals a day and snacks like muffins in between. She even made her own group of friends with the other residents at the Protestant home and they did numerous activities to keep everybody involved. My mother passed away in November of 2010, but I know that she enjoyed the last few months of her life. She was surrounded by her new friends, whom she referred to as her family. I was able to visit her whenever I wanted and they kept us very involved in her daily schedules and medical problems. I am glad my mother had the experience that she did at the Protestant Home, because it changed the way that I think about retirement homes.
However, and despite its growing importance, expenditure on home healthcare makes up less than 5% of that amount. Her experience spans almost 30 years in nursing, starting as an LVN in 1993. She has worked in Medical-Surgical, Telemetry, ICU and the ER. She found a passion in the ER and has stayed in this department for 30 years.
The fundus should be firm to compress the bleeding vessels at the placenta site. We share with the social care sector a vision for personalisation - all who receive support, whether statutory or self-funded, should be empowered to shape the services on which they draw. The aim is to transform social care, including housing, benefits, leisure, transport and health needs. We understand that people need quality help and support, close to home and at the right time.
We don't simply juggle people around on an impersonal plan! Even when our staff have holidays or other leave we make sure that those who call instead are known to the person they are visiting. Breastfeeding is often encouraged immediately as it stimulates the oxytocin reflex which can stimulate uterus contraction, reducing bleeding. Educate and encourage the mother to request to breastfeed if they desire. Restlessness, tachypnea, and tachycardia are symptoms of anxiety but are also symptoms of PPH.
Surgical management may include evacuation of hematoma and ligation of a bleeding point, laceration or episiotomy extension, D&C, abdominal hysterectomy, or bilateral ligation of the hypogastric artery. Ask the client to void first before performing the massage, as an empty bladder prevents displacement of the uterus and ensures accurate assessment of uterine tone. With a gloved hand, place one hand on the abdomen just above the symphysis pubis and another hand around the top of the fundus to anchor the lower uterine segment. Do not overly massage because the excessive stimulation to contract it will tire the uterine muscle and worsen uterine atony. Once the uterus is firmly contracted, it should be left alone but still assessed regularly. Investigate reports of persistent perineal pain or feeling of vaginal fullness.
